The answer so far

All three contain semaglutide, but they are different products. They come in different forms and have different approved uses. A finding about one product does not automatically apply to another.

Ozempic, Wegovy and Rybelsus are different semaglutide products. In the US labels cited here, Ozempic and Wegovy each have injection and tablet forms; Rybelsus is a tablet. Their approved uses differ, and a result for one product or formulation does not automatically apply to another. Availability and approvals can differ by country.12345

Same molecule, different intervention

A medicine is more than its active ingredient. Studies may use different doses or forms, such as a tablet or injection. They can also involve different groups of people, comparisons and follow-up periods. Saying only “semaglutide” can hide those differences. See why the form of an oral peptide matters.12

What the product records say

As of the cited 2026 US labels, Ozempic is available as an injection and as tablets. Its tablet label describes blood-glucose control in adults with type 2 diabetes and reduction of major cardiovascular events in specified adults with type 2 diabetes. The injection has its own label and additional use-specific indications. Rybelsus is a tablet; the cited US tablet label gives indications for type 2 diabetes and cardiovascular risk reduction in specified adults. Rybelsus and Ozempic tablets are not interchangeable milligram for milligram.25

The naming also changed: in January 2026, FDA approved changing the name of the R2 formulation of Rybelsus to Ozempic tablets. This applies to that specific formulation in the US approval, not every Rybelsus product in every country. Check the name, strength and formulation with the prescribing clinician or pharmacist before changing medicines.9

In the UK, the MHRA warned in December 2025 about a different change: newer Rybelsus tablets are absorbed more readily and have lower labelled strengths than the older formulation. Confusing the formulations or trying to match the old milligram strength with extra tablets can cause an overdose. If the tablets or strength look different, check the actual product with the pharmacist or prescriber; do not adjust the number of tablets yourself.10

What is Wegovy authorised for?

The cited US label describes both Wegovy injection and tablets for chronic weight management in specified populations, alongside reduced-calorie diet and physical activity, and for cardiovascular risk reduction in specified adults with established cardiovascular disease and obesity or overweight. The injection label also covers adults with noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) and moderate to advanced liver scarring. This liver indication received accelerated approval based on improvements in MASH and fibrosis; continued approval may depend on a confirmatory trial demonstrating clinical benefit. The tablet label does not list that liver indication. These details differ by formulation, population and jurisdiction; consult the current label for the product and use being considered.4

In March 2026, FDA also approved a higher-dose injection called Wegovy HD for weight loss and long-term maintenance in specified adults with obesity or overweight plus a weight-related condition. This is a distinct US approval context, not a recommendation to change treatment. The FDA reported that altered skin sensations occurred more often at the higher dosage. See the semaglutide profile for further context.8

What a trial result can answer

A randomised trial estimates average effects in its enrolled population under its protocol. It does not prove that every semaglutide product has the same effect, that an unapproved preparation has the same quality, or that a result for weight, glucose or cardiovascular events answers the other questions.

For example, STEP 1 studied a particular weekly injection in 1,961 adults with overweight or obesity but without diabetes, alongside lifestyle support. At 68 weeks, average weight change was −14.9% with semaglutide and −2.4% with placebo. Nausea and diarrhoea were common; 4.5% versus 0.8% stopped treatment because of gastrointestinal events. Novo Nordisk funded the trial. The figures are group averages, not a promise for an individual.

SELECT asked a different question in adults aged 45 or older with established cardiovascular disease and overweight or obesity, but no diabetes. Over a mean follow-up of about 40 months, cardiovascular death, nonfatal heart attack or nonfatal stroke occurred in 6.5% receiving weekly semaglutide and 8.0% receiving placebo. This is an absolute difference of 1.5 percentage points in that trial, not proof of the same benefit for people without established cardiovascular disease or for all formulations. In SELECT, 16.6% versus 8.2% permanently stopped the trial product because of adverse events. Novo Nordisk also funded this trial.67

A careful way to compare them

Before comparing claims, check the product and country, what it is approved for, and who was studied. For a study, also check what treatment it was compared with, what was measured and for how long. Missing details can make a comparison misleading.167
